Note: You can now etransfer your membership fees to the email address on the form. Once you have sent your e-transfer write the transaction number on the form (to allow the membership coordinator to match your payment to your form). Sign the waiver on the second page. Scan both pages and email them to the membership coordinator (email address is the same as for e-transfers).
